Meet with client / family group etc. to discuss requirements.

(Consult time of up 2 Hours allowed for)

- Needs Assessment

- Advise on available service/s and how to registering with them

- Advise on what service/s you can provide

 

Meet with client to:

- Register with Aged Care Services & Providers

- Advanced Care Directive planning if required

- Form completion assistance

- Assistance with updating other documents such as Enduring Power of Attorney

- Any other agreed upon requirements

 

Visit with client at specified location for:

- Companion visit

- Provide avenue for family time via Skype / Facetime etc

- Outside appointments

- Social visits & outings 

 

Provide in-home massage to client - part or full body

- massage chair or table available

- specifically blended oils available

 

Provide a foot care session for client

- trim & file nails, remove hard, dead skin build ups

 - provide a stimulating massage while moisturising foot and ankle areas

 

Respite services within the clients own home can be arranged

- dependent on individual needs

 

Attend hospital to stay with client to assist with:

- Client being attended to in timely manner

- Family contact to provide updates

- Keeping client as calm & comfortable as possible

- Explain procedures to client if needed

- Checking on any Pets if required (see below)

*After Hours Call Out Fee May Apply Between 9pm & 7am

 

Client is unexpectedly away from home eg emergency trip to hospital.

- Short duration - pet/s (small pets - dog, cat, bird) to be checked,

fed & watered if required, client advised

- Longer duration - as per above, arrange longer term care as per client

or family request

 

Work with client / family to reduce clutter / hoarding within home

- Paperwork / cupboards / whole of house

- Re-organisation of items into filing systems, archive boxes etc.

- Document organisation -sort into Save, Shred,  Remove & Recycle

- Assist family with Deceased Estates re clearing, sorting etc

- Any other requirements in this area

 

Consult with client in regards to requirements:

- Liaise with removalist/s for quotes, packing, storage, uplift & delivery

scheduling etc

- Work with client regarding pack & storage needs eg De-cluttering etc

- Assist family with Deceased Estates re clearing, sorting etc

- Any other requirements in this area

 

For clients needing to attend appointments, clinic visits or events

- Meet with client at home and assist to get ready if required

- Travel to destination

- Assist client with any logistical requirements - mobility aids, vehicles,

venue assistance

- Personal care requirements

Return journey and assist client with any care needs before departing

 

Assist client with all aspects of travel as per above but on longer time scale:

- Transit through airports or terminals, transfers to accommodation

- Accommodation check-in etc

- Personal care requirements

- Meal time assistance

- Medication reminders, etc

- Travel needs whilst on trip - to points of interest, meetings, family events

- Making sure the client enjoys their time away!

 

For Services that require extended hours or 2+ days eg:

Travel Companion, Move Management, Decluttering, Respite etc

A Quotation will be provided

Purchases made on behalf of client
